To get credit on any project, you must completely fill out the Certification of Originality, Attribution, and Disclosure. To download it, click on this link to download the file in Word format.
Fill out all portions of the document, adding lines if needed. If a section does not apply, do not remove it. Instead, indicate “N/A” (not applicable). Fill it out completely and truthfully, erring on the side of disclosure.
A submitted project cannot receive any score at all until and unless the student provides a timely and complete, signed, and dated certification form. If the form is lacking or is facially incomplete, I will notify the student and give them an opportunity to timely correct the document. Cf. FRCP 11(a).
